Important!

Any new account or password reset/account unlock request will require you to have completed the WesternU HIPAA Training for the current academic year. 

If you do not have a completed training on file, your request will be cancelled and you will need to resubmit a new request upon completing the HIPAA Training linked above.

Available Services:

Please review the below and select the relevant option on the right-hand side of this page to submit your request:

Employee Request:

  • For any WesternU Health employee needing access to Epic or other ancillary systems (Onbase Scanning, Remote Portal, etc), or requiring assistance with logging in (password reset, account unlock/unblock).
  • Requires being logged into support.westernu.edu.
  • CDM/Dental Center employees will need to contact cdmit@westernu.edu for access to the Dental Center(s) specific clinical systems.

Student Request:

  • For any WesternU student needing access to Epic as part of their upcoming clinical rotation, or requiring assistance with logging in (password reset, account unlock/unblock).
  • Requires being logged into support.westernu.edu.
  • CDM students will need to contact cdmit@westernu.edu for access to the Dental Center(s) specific clinical systems.

External User Request:

  • For any student extern or non-WesternU/part-time employee needing access to Epic, or requiring assistance with logging in (password reset, account unlock/unblock).
  • For access to the Dental Center's systems, please contact cdmit@westernu.edu.

 

Additional Information:

  • This form can be used to request a new Epic account as part of your upcoming or future WesternU Health Rotation 
    • Select "Submit Request" on the right side of this page to submit your request.

  • Students with existing/previously issued accounts that need their password reset, or account unlocked, can also submit a request to have their access restored prior to a new clinical rotation.

  • This form is for requesting access to Epic at a WesternU Health facility only
    • WesternU does not manage access to the health records systems at other clinical sites you may rotate through.

Please be aware, account requests may take up to 2 business days to process.
