Important!

Any new account, or password reset/account unlock request will require you to have completed the WesternU HIPAA Training for the current academic year (2022 - 2023). 

If you do not have a completed training on file, your request will be cancelled and you will need to resubmit a new request upon completing the HIPAA Training linked above.

Additional Information:

  • This form can be used to request a new Epic account as part of your onboarding. 
    • Select "Submit Request" on the right side of this page to submit your request.

  • Employees with existing/previously issued accounts that need their password reset, or account unlocked, can also submit a request to have their access restored.

  • This form is for requesting access to Epic at a WesternU Health facility only
    • WesternU does not manage access to the health records systems at other clinical sites you may be affiliated with.

Please be aware, account requests may take up to 2 business days to process.
