Epic Remote Access Request

Please note:
- This form is for access to the remote portal only; an (additional) Epic account is still required to access the EHR.
- Students, Staff, and Providers will have needed to complete the mandatory HIPAA training located here prior to submitting this form.
- Student Remote Access MUST be approved by a Clinical Faculty member.
- Failure to receive approval from the supervising provider/preceptor will result in a denied request.
