Please note:

  • This form is for remote access only (not HIPAA training)

  • Students, Staff, and Providers will have needed to complete the mandatory HIPAA training located here prior to submitting this form.

  • Student Remote Access MUST be approved by a Clinical Faculty member. 
    • Failure to receive approval from the supervising provider/preceptor will result in a denied request.

Please be aware all access requests can take up to two business days to process.
