Getting Started with Epic: Document Patient Screenings

Screenings Activity

  • Clinical support staff may use the Screenings section to quickly document standard patient screening questionnaires when rooming a patient as an alternative to using the Flowsheets activity.
  • From the Patient Encounter:

1. Click on the Rooming tab.
2. Click on the Screenings section.

  • If the encounter type does not have the Screenings activity or Rooming tab, this activity can be selected from the More menu (down facing triangle icon).

3. Click on the specific screening or combo screening available.

  • You may select multiple questionnaires or screenings at a time.

4. The selected screening questions will appear below. Use the speed buttons or checkboxes to select your answers. Buttons turn blue when selected. Click again to deselect the answer. 
5. For screenings with scores, the score will auto populate at the bottom of the questionnaire when all questions have been answered.
6. Click the comment button (paper document icon) to write a free text note for any question.
7. Rows with comments display the icon with lines. Hover over the comment icon to view its content.

8. When answered a certain way, some questions will prompt a cascade of other questions.
9. Responses with exclamation icons indicate abnormal responses.

10. Click the flag icon to mark a response as significant.

11. Once the screenings have been completed, click Close and the bottom of the section.

12. Click Show Last Filed Value to view historic patient responses.

  • Patients 12-17 year old will be presented the “BH Adolescent Screenings” that offers CRAFFT instead of AUDIT and DAST, and those 18 years or older will receive “BH Screening” that offers AUDIT and DAST.

13.  When the GAD-2 screening is completed, a score over two is flagged in red with an exclamation mark. To save clicks when GAD-2 is selected the GAD-7 is expanded. Any combination screenings that include the GAD-2 screening will behave in the same way.
